Blakdyak was found dead inside his rented studio-type apartment on 21 November 2016. He was 46. He was found at 6 p.m. by his young son naked and with a plastic bag around his head. His son, who wondered why his dad still had not opened his room, decided to peer from a window to check. Police have yet to determine whether there was foul play in the comedian's death.

According to Esteban Rubio, the apartment's security guard, Formoran had climbed to the apartment's rooftop while in a paranoid state the night before. He reportedly went back to his room afterwards. He added that no one could have intruded in Formoran's death as people are required to register before entering the apartment's premises.

Formoran's wife Twinkle, who's still in a state of shock after hearing the news, said that they did not have any problems. She lives seperately from Formoran with two of their three children. Their eldest child lives with Formoran.

In June 2014, he got arrested after allegedly creating trouble in an apartelle in Cubao. In October of that same year, he also faced drug charges. He was able to pick himself up and resume his career by headlining shows and gigs. He was collaborating with veteran composer Lito Camo for a new album.
